Customer Information We Collect & May Disclose
The purpose in collecting personal information is to efficiently provide financial products and services to meet your needs. We collect the following types of non-public personal information from various sources:
- Identification information, such as name, address, telephone number, and Social Security number;
- Information we receive from you on applications for loans or other forms, such as assets, income and debt that will help us determine if you are eligible for products you request;
- Information about your transactions and account experience with us, such as account balances, payment history, account usage, and your inquiries and our responses;
- Information we receive from a consumer- reporting agency , such as credit score and credit history;
- Information from outside sources regarding employment, credit and other relationships that will help us determine if you are eligible for products you request, such as employment history, loan balances, property insurance coverage, and other verifications.
Customer information is used for several reasons including servicing a customer's account, processing applications, suggesting other products and services that may be beneficial to the customer, and as required by the USA PATRIOT ACT to verify your identification.
We may collect personal health information, like medical reports, for certain types of insurance policies. We do not share personal health information with anyone except as necessary to underwrite the policy, administer claims, or as otherwise requested by you or as required or permitted by law.
Safeguarding Your Information
United Bank takes the greatest care and precautions to assure you that any of the non-public information we receive is used within strict guidelines. Our employees have limited access to customer information and are permitted to access this information only when it is necessary to conduct business and complete transactions. Information is maintained by physical, electronic, and procedural safeguards in compliance with federal regulations to guard your nonpublic personal information.
Visitors to the United Bank web site will remain anonymous. We do not collect personal identifying information about visitors to our site. If you have provided identification (such as name and address) via e-mail or information request forms, the information will only be used to communicate with you to handle your request. It is not sold or transferred to other parties. Beware of phishing, pharming schemes, etc or of any attempts to steal your identity. Do not respond to e-mails requesting account numbers, PINs or passwords; we do not request these types of information from our customers via e-mail. Please call us to report these types of requests.

Restrictions on Sharing Information with Third Parties
It is our policy not to sell, transfer, or otherwise disclose any non-public personal information of current or former customers to outside companies, except as permitted or required by law or as authorized by you; such as:
- The customer requests and authorizes the release of information
- Information provided to service the customer's account
- The information is required or permitted by law (to report fraud or criminal activity, in response to a subpoena)
- To report to credit bureaus
We may provide customer information to outside organizations that provide us services (such as printing or mailing) or help us provide you with products or services you request (such as brokerage transactions or underwriting insurance applications). Consumer's Right to Opt Out
If you prefer that we not disclose nonpublic personal information about you to nonaffiliated third parties and affiliated companies, you may opt out by directing us to not share nonpublic personal information (other than disclosures permitted by law). If you wish to opt out of disclosures to nonaffiliated third parties and/or affiliated companies, you may contact us by:

If you have a joint account, we will treat a request not to share nonpublic personal information by one account holder as applying only to that account holder unless the customer notifies us that he/she is acting on behalf of the other account holder(s). If you have already requested that we not share this type of customer information, there is no need to contact us.
